 Guidelines for Faculty / Staff Memberships to the

Student Union  Fitness Club

Rates:

$240.00: Annually

$100.00: Per fall and spring semesters

$40.00: Summer

1. Faculty and staff members can purchase Fitness Club memberships for the following time periods:
   1. Annual membership: $240.00
   2. Fall semester only: $100.00     
   3. October 15, 2007 – December 12, 2007: $50.00
   4. Spring semester only: $100.00
   5. March 15, 2008 – May 6, 2008: $50.00
   6. Summer only: $40.00

2. Faculty/staff members can choose from two different payment options when purchasing their Fitness Club membership.
   1.  Pay their membership fee in full with cash or check at the Cashier’s Window, Reng Student Services Center Room 2146.
   2. Use payroll deduction to pay for the membership.
3. Paying membership in full:
   1. There will be no refunds given for the Fitness Club membership.
   2. There will be no cancellation of the Fitness Club membership.
4. Payroll Deduction:
   1. Your payroll deduction will be set up by Finance so that the total amount of your membership fee will be divided by the amount of pay periods that are left in your membership period.
   2. There will be no refunds given for the Fitness Club membership.
   3. There will no cancellation of the Fitness Club membership.

5. To purchase a membership:
   1. Please make your payment at the Cashier’s Window (second level of the Reng Student Services Center).
   2. The Cashier’s Window will give you a receipt which should be taken to the Student Account Services office which is next door.
   3. The Student Account Services office will add access to the Student Union Fitness Club onto your faculty/staff ID upon seeing your receipt of payment.
6. After purchasing this membership you will have access to all Fitness Club amenities and classes for the length of time that your membership was purchased.
7. You will not be allowed admittance to the Student Union Fitness Club without your ASU faculty/ staff ID.
